California sober usually means no alcohol and no hard drugs, with cannabis, and sometimes psychedelics, staying. It is harm reduction with a nickname. Whether it works depends entirely on what was stopped and what was kept, and for one class of drugs the arithmetic is dangerous enough to say plainly.
It spread in 2021 when a well-known singer used it for their own approach and later publicly walked it back. Before and after that, people had been doing the same thing under no name at all: give up the thing that was wrecking your life, keep the thing that was not.
For some people who drank heavily, cannabis instead of alcohol means fewer blackouts, fewer fights, a liver that gets a break. The evidence on cannabis as a substitute for alcohol is mixed: some studies find drinking goes down, others find cannabis use disorder goes up in its place. The honest test is not the label. It is whether the thing you kept is now doing the job the thing you stopped did.
Opioids. Any return to opioids after weeks away meets a body whose tolerance has dropped, and cannabis does nothing to protect against that. If the thing you stopped was fentanyl, heroin or pills, then California sober is only safe if opioids are on the stopped side and stay there, with naloxone in the house regardless.
Buprenorphine, methadone and naltrexone are not substitution in this sense. They are treatment, they cut the risk of dying from opioids by about half, and being on them is being in recovery. Marrow has a room for medicine that says exactly that.
